40 year old female who had a previous breast augmentation and was very unhappy with her result. She had a saline subglandular augmentation and her implants have resulted in severe rippling and her implants have ended up under her arms. She has a lack of central fullness and her implants end up on her side when she lays down. We did a breast augmentation revision with a complex surgical approach. A switch to larger silicone gel implants along with a pocket change to subpectoral was done. The pocket change utilized AlloDerm® dermal matrix to define the pocket and reduce rippling postoperatively. This resulted in a dramatic improvement and has almost completely eliminated her rippling….

This also resulted in visible rippling at the top of her breasts even though her implants were under the muscle. • Has performed thousands of breast surgeries during his 15+ years of surgical experience

How will I choose the right implant size for me?

Choosing the size that will meet your cosmetic goals can seem like a complicated process. The same size implants that look great on one person may not be the best option for you. That’s why it’s so important to choose a plastic surgeon such as Dr. Smith who uses his extensive experience and sense of aesthetics to help patients in Orange County choose the breast implants that suit their cosmetic goals and their lifestyles.

Dr. Smith makes the process simple and even quite fun. Patients can see and feel different styles of implants during their consultations, as well as review before-and-after photos to get a sense of what implants will get the results they want for their body types.

Additionally, Dr. Smith uses the VECTRA® 3-D imaging system, which simulates how you’ll look with various sizes of implants. This system lets you manipulate real images of your body to see how different options may look. Many patients say this process helps them feel confident in their choice of implants so they approach their surgery date feeling eager and excited.

Are silicone implants really safe?

Silicone breast implants are some of the most studied medical devices in history. The Food and Drug Administration re-approved them for use in women who are at least 22 years old in November 2006, after 14 years of hearings, studies, and reports.

Hundreds of thousands of women around the world have received silicone implants since the 1960s, and in fact, while the devices were restricted in the U.S. they were never banned in other countries. No credible medical evidence has ever come to light linking the implants to serious complications.

How much pain can I expect after surgery?

Most people report experiencing moderate discomfort for the first 1 or 2 days after breast augmentation. Dr. Smith prescribes pain medication; you should fill the prescription and have over-the-counter pain relievers on hand prior to surgery. After the first couple of days, discomfort should be mild. You may experience some pain for several days when you use your arms and chest muscles.

You may notice some bruising and swelling. Some patients report pressure and stretching in the breast area. Ice packs can help.

How much time off work should I take?

If you’re like most patients, you will probably feel ready to return to a fairly non-strenuous job in 3 days or a bit longer.

Are there other preparations I need to make?

You should have someone ready to take you home after surgery and someone to help you at home for a couple of days. You may need assistance with meals, children, and pets.

You should also prepare your home with your comfort in mind. You may want to have ice packs on hand to help with swelling and you might be most comfortable with clean linens and extra pillows on the bed.

When can I resume physical activity?

It is important to start walking within a day of surgery to promote good circulation, which enhances the healing process and lowers the risk of complications. Wait about 2 weeks before starting light exercises and at least a month before resuming more strenuous aerobic workouts that include jumping or running. If you lift weights as part of your exercise routine, you can begin light upper body workouts after 2 months.

Will my nipple sensation be normal after surgery?

It is not uncommon for sensation to be reduced during the recovery period. Actually, many patients state that sensation is increased and some find they have asymmetric sensation. Very rarely a patient experiences some permanent loss of sensation.

How long should I wait before going bra-less?

Dr. Smith advises most of his patients to wait several weeks before going without a bra.

How long will I need to refrain from sexual activity?

When your breasts have had 2 or 3 weeks to heal and the soreness and sensitivity go away, you may cautiously resume physical contact and direct stimulation of the breasts.

Will I be able to breastfeed after breast augmentation?

None of Dr. Smith’s patients has ever reported not being able to breastfeed, although there is no absolute guarantee.

Do breast implants affect my risk of breast cancer? Will I still be able to get mammograms?

Breast implants will not change your risk of breast cancer and you will still have mammograms as usual. Since implants change the feel of your breasts they can change the sensitivity of breast examination. It is important to inform the technician that you have implants before mammography. If you are over 35 years of age, a baseline mammogram should be performed before your surgery.

How long will my augmentation results last?

It’s possible to have breast augmentation and never have another procedure. It is common, however, for patients to come back to replace a ruptured implant, change to a different kind of implant, or seek a breast lift to maintain their youthful shape.

Dr. Smith understands your desire to look your best, and is happy to answer all of your questions in detail at your personal consultation.
